Introduction

The Patient Access Network (PAN) Foundation respects that the privacy of your personal information is important to you. We have developed this policy to inform you of how the Information (as defined below) that we collect about you will be used. In the event we make changes to this policy, we will post the revised policy on this website together with the date on which we made the most recent changes. We ask that you revisit this page periodically to be sure that you are familiar with the current version of our policy.

Collection of information

PAN may collect personally identifiable information about our patients, such as name, address, telephone number, e-mail address, income, or other similar information (“Information”) when you voluntarily submit this information or when you give permission to others to submit this information on your behalf.  

Information submitted under “Apply Now” is only used for pre-screening to determine whether you meet the eligibility criteria for assistance from one of our disease funds.  The information is not stored in our databases until you submit an application.

Through our websites, PAN also collects non-personally identifiable information related to such matters as: the total number of visits to our websites, the number of visitors to each page of our websites, the Internet Protocol (IP) addresses of our visitors, and the time spent on our websites. We use this information to understand how our visitors use our websites so we can improve the customer experience. We may also use your IP address to help diagnose problems with our servers, and for purposes of system administration. From time to time, we may also share non-personally identifiable information with others.

We gather aggregate data about visitors to our websites for purposes of improving our websites, and also for market analysis. We may provide information from our websites in aggregate form without personally identifiable information to third parties. We do not sell this or any other information we collect to third parties. Any third party that receives aggregated data must agree not to attempt to re-identify the persons to whom it belongs.

If you contact us to report on your experience, or the experiences of your patient, with the use of our service, we may contact you for follow-up information.

PAN may share any or all of the above information with its third-party contractors, so that its contractors can use your information in furtherance of services performed for PAN. PAN also reserves the right to share your Information in order to respond to authorized information requests of government authorities, the courts, law enforcement agencies or where otherwise required by law, for corporate audits, or to respond to complaints or security threats.

PAN uses reasonable technical and administrative safeguards to protect Information from unauthorized access, use, loss or destruction. Please be advised that although we have tried to create a secure and reliable website for you, the confidentiality of any Information, or communication or material transmitted over the Internet cannot be guaranteed. We urge you to exercise caution when conveying personally identifiable information over the Internet. When disclosing any personally identifiable information, you should remain mindful that such information might be accessible by the public and, consequently, may be collected and used by others without your consent. Your use of the Internet and this site is solely at your own risk.

PAN’s website uses security precautions to ensure the highest level of protection possible that is currently available to PAN. We have a Secure Sockets Layer (SSL) Certificate, also called a Digital Certificate, which creates a secure link between our website and a visitor’s browser. SSL encryption ensures that all data passed between the two remain private and secure, preventing hackers from stealing private information such as credit card numbers, names and addresses.

Donation Security and PCI DSS Compliance

PAN uses the highest possible security measures for online donations currently available to it. PAN’s donation pages service provider is PCI-DSS-compliant and is PCI-1-Level-1-Certified. PCI- DSS compliance standards require strict adherence to a large number of security requirements to maintain our certification. All servers, workstations, network connections, products and offices are audited annually by a third-party PCI-DSS-qualified security assessor.

Cookies

PAN does not use cookies.

Minors

We do not knowingly collect directly any personal information from anyone under the age of 13, nor do we knowingly market our websites to anyone under the age of 13.

As a convenience to you, PAN may provide links from this website to other websites maintained by independent third parties. This policy applies solely to the information collected on PAN’s website.
