Washington, DC, January 7, 2016 – The Patient Access Network (PAN) Foundation today announced the launch of a new patient assistance program for Medicare patients being treated for hypercholesterolemia. Qualifying patients are eligible to receive grants for the out-of-pocket costs associated with their prescribed hypercholesterolemia medications.

“With the launch of our new hypercholesterolemia program, the PAN Foundation is pleased to offer three assistance funds related to the prevention and treatment of heart disease,” said PAN Foundation President and CEO Daniel Klein. “Nearly 27 million people in the United States are living with heart disease, and PAN is committed to easing their financial burdens so that they can access the treatment they need to help manage their arrhythmia with atrial fibrillation or atrial flutter, heart failure and, now, hypercholesterolemia.”

Patients who qualify for financial assistance from the PAN Foundation for their prescribed treatments for hypercholesterolemia, arrhythmia with atrial fibrillation or atrial flutter, and heart failure must have Medicare insurance and must meet the following criteria:

Table for assistance programs, maximum award level, and income eligibility for hypercholesterolemia, arrhythmia, and heart failure

“Ensuring patients have access to the resources they need to improve and maintain their cardiovascular health is essential,” said Dr. Martha Gulati, Editor-in-Chief of CardioSmart.org, the patient education and empowerment initiative of the American College of Cardiology. “Heart disease is the leading cause of death for both men and women in the United States, and by offering a variety of heart-disease-related patient assistance funds, the PAN Foundation is addressing a critical need.”

Patients, or advocates and caregivers applying on their behalf, can apply for assistance using PAN’s patient portal (https://www.panapply.org/) or by calling 1-866-316-PANF (7263), 9 a.m. to 5 p.m. Eastern Standard Time, Monday through Friday.
